To receive free shipping:

1. Click the following link to open the Order Form.
2. Click on the "Print this page" link at the top of the form.
3. Fill out the top portion of that page, including phone number - so we can reach you.
4. Cut the mailing label (bottom portion in red) off the order form.
5. Put your repair in a bubble mailer or a safe shipping box along with the top portion of the order form.
6. Attach the mailing label to the package.
7. Give package to the mailman.
8. We will follow through with an estimate if requested.
